Our commitment to your privacy
Shades of Insight Therapy is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms currently in effect.
How we may use and disclose your information
We may use and disclose your PHI for the following purposes without your written authorization:
- Treatment — to provide, coordinate, or manage your care, and to consult with other treating providers when appropriate.
- Payment — to bill and collect payment for services, including submitting claims to your insurance.
- Healthcare operations — for quality assurance, clinical supervision, and other activities necessary to run the practice.
Other uses and disclosures — including to family members, for research, or for marketing — require your specific written authorization, which you may revoke at any time.
When disclosure is required without authorization
In limited circumstances, we may be required to disclose information without your authorization: when required by law, to prevent serious and imminent harm to you or others, in cases of suspected abuse or neglect, or in response to a valid court order.
Your rights
You have the right to:
- Request access to and a copy of your health records
- Request an amendment to your health records
- Request an accounting of certain disclosures we have made
- Request restrictions on certain uses or disclosures
- Request confidential communications by an alternative method or location
- Receive a paper copy of this notice, even if you agreed to receive it electronically
- File a complaint if you believe your privacy rights have been violated
Filing a complaint
If you believe your privacy rights have been violated, you may file a complaint with our practice directly, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
Changes to this notice
We reserve the right to change this notice and to make the revised notice effective for information we already have as well as information we receive in the future.
